When I place pistons in a specific way, I can get an extended piston with a piston head coming out the side. That head can't be broken unless I place a piston facing the same direction, power it, then break the piston. Also, if it's a sticky piston and you place a block in front of the head, the head disappears with the block taking its place.
It happened when I placed a piston next to a redstone block and moved my cursor while doing so, and it just happened.
