Happy ghastlings seemingly randomly despawn while on leads or nametagged, just like on Bedrock edition and its dedicated servers/realms. I additionally play with subtitles and can confirm that a death noise did not play when the happy ghastling despawned.
Motions That Occurred During the Issue
Tied the (unnametagged) ghastling’s lead to a fencepost.
Right clicked bed nearby the ghastling to skip to morning.
Ghastling had disappeared, and soon a lead dropped.
Additional Notes
It has been posted on forums that this has been occurring while nametagged as well, but I did not lose the other three ghastlings I raised in my world. I’m not sure if sleeping near the ghastling triggered it, as I tried to repeat my steps with another ghastling and it did not disappear (albeit this time nametagged to see if the issue persists with it).
I find it odd that the same bug is occurring on both platforms despite different coding languages, but that’s not to say it isn’t possible.
Edit: Possibly related to the same issue so editing instead of opening a new report; happy ghast leads are also popping off the ghast (without snapping or the snapping audio playing), dropping the lead and allowing ghasts to escape. This could have happened earlier, and the ghastling flew high enough that it simply despawned.
Edit2: Leads are also popping off when tying entities to a happy ghast, so tying a boat to a fence post and a ghast to the boat does not work as a consistent workaround.

Sure thing! When I initially sent the report I wasn’t sure what was causing it; I was mostly baffled hence just providing the steps I took before the issue.
I do have a bit more information now from messing around and have a clearer idea of what the steps potentially are. If I get a recording I'll provide that as well.
Steps to Reproduce
Pick an open space without hazards to rehydrate a happy ghast.
After rehydrating a happy ghast, tie the ghast(ling) to a fence post or an entity. If tying to an entity, ensure its a chained lead and the entity does not become a passenger.
Sometimes, the lead will “pop.” It does not snap, no snapping audio plays. The lead will drop to the ground and will not be lost, but the ghast(ling) will begin to fly away.
Unnamed/unharnessed ghast(ling)s, will fly out of the player's range, causing them to despawn (“disappear”).
Observed Results
Ghast(ling) does not stay tied to a lead. This does not happen consistently and will sometimes occur back to back to back or not at all. The leads will “pop” with both harnessed and unharnessed ghasts.
Expected Results
The ghast(ling) remains tied to the lead until a player unties it or the lead snaps (I.e. a creeper blows up what its tied to, the ghast(ling) dies, etc).

I created a flat world with a 100x100 grid of ghastlings and was ultimately unable to recreate the bug. Additionally, the ghastlings didn’t fly more than 20 blocks up during my trial run, so I’m not longer certain that is the cause.
My render distance was set to 28 during normal gameplay, and 32 for the rest world.
Regarding answer 1, upon leaving one intentionally lose to see if it would despawn out of nowhere, I noticed it flied up, but it did not exceed that height. The flat world was created at y=-64 so it wasn’t a matter of hitting the build limit.
