What I expected to happen was: I press left-shift to exit a minecart and I end up standing next to it.
What actually happened was that I dropped through the ground and fell until I hit something else. I have train station platforms in the Nether that are suspended in the air and are one block thick but have a complete tunnel with roof and sides and at least one row of blocks next to the tracks at all points. The spots I fell through (it's reproducible and appears to happen every time) are made of netherack. Any exiting along this suspended 1-block thick track platform causes me to go through it. At one station I go through the platform, then hit air, then hit netherack maybe 20 spaces down. At the other station I fall maybe 40 spaces down into a lava lake. The player is using a skin file and was wearing armor. The game is not modded.
Steps to Reproduce: No way to test all combinations on my own, but what seems to reproduce it is to get into a minecart on a track that only has one layer of blocks holding it up and then air below and then exit--I consistently go through the platform blocks instead of standing on them.

I also discovered this problem on a multiplayer server.
For me it happend also in the nether. The tracks are inside a tunnel, 1 block free between the tracks and the walls.
When i exit the minecart when the tunnel is above a lava see, I fall into that sea. When i exit when the tunnel is on solid blocks, I find myself standing outside the tunnel walls.
So my suggestion of what happens is that the game can't place you directly besides the tracks and so it takes the next free space beside the track which is outside of the tunnel.
Since 1.7 the player's jump out of the minecart is longer. The landing can be up to 3 blocks away in both the x and z direction, in the nether, but also in the upper world it is farther than before. Even if there are open spaces nearer to the cart.
The player is also catapulted through glass walls in the nether, without breaking the glass.
Jumping through walls, floors and glass without damaging the walls or the player is almost certainly just a bug.
The longer jump out of the minecart could be a bug or a new feature, but a very annoying one for all who have to rebuild structures.
What I think:
Probably the landing is miscalculated, as it seems to try to land at x+3 and z -3 from the minecart. I have not seen much happening in the z direction, but maybe just coincidence that I have no thin floors where I want to exit. Or else the exit-function searches for open space and all the blocks nearby are marked occupied. Good luck!
Learning this interface. Picture got posted by itself. Will post a few more pictures to explain. I managed to reproduce this issue at will. I built a tube three blocks wide, three blocks high, and enclosed (so it's really 5 x 5 in cross section). I put railroad track down the middle. I dug a two-block pit (requiring putting a few blocks underneath the main tube) and put two powered rails and a button on a block next to it. If I sit in the cart and exit, I always fall through the tube into air, even though there are plenty of blocks on the inside. Even putting an extra row of blocks underneath the pit for the cart stop didn't change the result.
I came to confirm this. What Mark Veldhuis said appears to be true: When you exit your minecart you are teleported instead of just coming out next to the cart. I fell through a wall and into lava in my survival world when exiting my minecart, not the best moment I can tell you. I have since confirmed it multiple times. Placing 3 rows of 2 high blocks behind the 1 block wall prevented me from falling through, but this cannot be a solution for such an obvious bug. If it helps I will make a video of it.
I've been having this issue all day. Anytime I exit a minecart, it tosses me a couple of blocks away instead of dropping me right where I exited.
Can confirm this is happening. On our multiplayer server it's quite annoying. Several people have died exiting a minecart in the nether, only to fall through the floor to their deaths.
I have also experienced this problem, but it is when a minecart is broken with a cactus. I fall straight through the floor.
