When you have a full hot bar, have a pair of sheers in your hotbar. Have the sheers selected and then use the pick block click on a bunch of blocks/items (That are not in your hot bar) and the sheers will not be replaced, and will become duplicated sometimes.
Edit: If tool replacement with pick-block key is not intended, I also tried doing the same with an enchanted diamond shovel and it was replaced. These duplicated sheers also appear to be ghost items.

Does this occur only with shears or also with other tools / blocks / items ?
It happens with all my other tools as well, except an enchanted shovel I have which was replaced by the pick block. I will experiment further current it duplicated iron picks, sheers, and irons swords. Sometimes you have to pick block multiple times.

Confirmed in 13w06a. Very strange.
I know this is not a very high priority bug, so I'm not expecting a fast fix XD. Thanks for confirming it though!
