Minecraft randomly freezes, then goes to a black screen

When playing the game, at random times it will just... Freeze. Then, when I alt-tab out to fix it, and go back in, the screen goes completely black and, while I can still move and place/break blocks, I can't see a thing.

One other note - This only seems to happen in Fullscreen mode.
