Dropping an Item and closing the World at the same time as you pickup the Item will let you duplicate the item.
How to reproduce:
1. Drop an item on the Ground below you
2. Wait until you pick the item up again
3. Hit Esc at exactly that moment (you should see the pickup animation of the Item in your Hotbar and still the Item on the Ground)
4. Exit the World
4.5 Some people are saying that you have to quit minecraft and relaunch it for the Bug to "work"
5. Reload the World
6. See that you have 2 Items in your Inventory

This is the screenshot from my reproduction of this problem in single player survival. That is only one block of wood on the ground, not more. The screen shot was captured as the object was being "pulled" towards me to go into my inventory and appears to be two items.
Please note that I have not been able to reproduce this problem every time even when following the exact same procedure that at other times, does successfully reproduce the problem. The attached image named "MC-71974_Reproduction-Failed.png" shows another attempt that failed to reproduce the problem even when the moment of the screen capture appears to be identical to others that succeeded. I have not yet figured out what variable causes it to reproduce at times and not at others. The "MC-71974_Reproduction-Failed.png" attempt results in only one item in my hotbar after reloading. Again, note that in this screenshot, that is only ONE piece of birchwood on the ground and not two, as the capture happened during the animation sequence.
Confirmed for 1.8.1. There was a lag spike which made me unable to pick a dropped pickaxe up (also prevents chunk loading, blocks from dropping items, entities from moving etc), relogging duplicated the pickaxe.
I have in fact been able to reproduce this but I had to modify the way I was doing it. The following steps reproduce the problem for me:
1) Start a single player survival game.
2) Chop one block of wood from a tree allowing it to "fall on you" so that it enters your inventory.
3) Drop the block on the ground.
4) Pick the item up and hit "Escape" at the exact same time such that you still see the item on the ground and the item appearing in the hotbar during it's "stretching" animation. This may take several attempts to get the timing right.
5) Click "Save and Quit to Title".
6) COMPLETELY EXIT THE GAME.
7) Restart game.
8) Reload saved game and observe two of the same items now in the hotbar.
Please note that if I do not completely exit the game and just reloaded the saved gamed from the Title Screen, I can NOT reproduce it.
