Essentially in creative mode, when you click an item and are able to move it around in your inventory, when you have a command testing for that item it will detect that its no longer in the inventory. However, in survival mode, when its hovering around, its not being detect as being not part of inventory.

that if you hold an item while in inventory (like changeing it's slot) testfor will not detect it in creative, but it will in survival
I guess this is intended so that changing an item doesn't cause restone to say the player doesn't have it (survival)
but that it doesn't detect it in creative because else it would detect every item in game❓

I apologize but I do not understand all of your wording in the description. Can you please elaborate? Exact steps for reproducing and screenshots of the before and after would be especially helpful.
