I suspected that if I have iron pickaxe with 1 durability left and fortune 3 I never get more than 1 diamond.

I believe I've confirmed this in 1.8.1-pre2, actually.
Gave myself an iron pickaxe with a damage value of 250 (one hit left), enchanted it with Looting III. Used it on a diamond ore block, it broke, and I only got one diamond. I repeated this multiple times, and not once did I get more than one diamond on the final hit of a pick's life, whereas Fortune III should have a 60% chance of getting at least two diamonds from a diamond ore block normally.
Here's a clip of me doing it five times in a row and failing to get more than one diamond.

So it definitely looks like Fortune enchants are not being applied to the final usage of pickaxes. Perhaps other tools too.
It's ALWAYS been this way. I'd call it a bug, but it's a very old bug. Fortune does not apply to the hit that breaks the pickaxe. Remember, in old versions you wouldn't get a drop from the block broken by the final hit of a pickaxe (in fact it would break partway through mining instead of at the end). They had to artificially add in a block drop as a workaround for the bug (which they never truly fixed). So the artificial block drop doesn't know about enchantments apparently.
The problem comes with that I want to make 1 use pick in adventure map where higher tear can drop more than 1 diamond.

I don't think you can get more than 1 diamond, even with Fortune. It probably only affects blocks w/several drops (such as wheat, coal, glowstone, redstone etc.)
