Can not replace item in off-hand slot.
Not sure is it a bug but i think it shouldn't
be in the game.

He might meant an issue which relates to MC-86947
World looks normal, but you sink through blocks, pressing F does nothing, you are forced to be right handed and maybe even more stuff like that.
I guess world sometimes can't see the player
