[MC-89235] Duplication bug with dying.

I writed /kill @e when I've got a filled map. Then I exited to the game, then
opened the game again. Then I selected latest server and in the game, the
filled map was in a hole. I filled the hole and then I have got 2 filled maps.

Please force a crash by pressing F3 + C for 10 seconds while in-game and attach the crash report ([minecraft|http://hopper.minecraft.net/help/finding-minecraft-data-folder]/crash-reports/crash-<DATE>-client.txt) here.
