I just made a diamond pick and went to use it. As I was using it the inventory slot started going green and then threw it on the ground. The second time it threw it it dissapeared and I cannot find it.

I have tried reseting the world, but as I clicked exit the game crashed. When I rejoined I spawned in the air above spawn and got a message that my home was obstructed. The house was perfectly fine but my pick was still gone

as you can see here (this is the second time this happened) the far right slot goes green before dropping the item in that slot last time I had the pick in that slot
