Beds respawn players in unsafe locations.

 When a player uses bed ( right-clicks on the bed), a respawn point is set where the player is standing at that moment. So, if you surround the area with obsidian or other hard blocks and fill the space with water, the player will drown forever every time he respawns.(Even though there are safe places around.)Underwater should not be selected as a respawn point unless the player has been granted water breathing or conduit power.

*P.S. The easiest way to fall into a death loop is to use the bed just before falling from a high place. This may happen more often than the above method.(At night you can end the loop by going to bed).

I can confirm that this does occur in 1.21.22, although rarely. There doesn't appear to be an exact way to reproduce this other than a lot of trial and error. I've found that the easiest way to reproduce it, is, as the OP mentions: you make sure to save your respawn point right before jumping off a cliff.

Steps to reproduce:

  1. Build a tower sufficiently high for you to die after jumping off it.

  2. On the top of said tower, build a 3x3 platform

  3. Place the bed on the ledge of said platform, so that there aren't any solid blocks adjacent to the position of where the player's head would be.

  4. Jump off the ledge and quickly right-click the bed to set your respawn point.

Observed results:
If you successfully trigger the cycle of death, you'll either respawn in the air straight away (like shown in the video), which I wasn't able to reproduce or what happened to me, which was that I respawned inside the edge of the bed, but it immediately pushes you off to the side effectively making you fall to your death.

Expected results:
You should always respawn at a "safe" position that doesn't lead to your death, which in this situation would be at the bottom of the tower.
