Particle emitters supposed to be bound to a locator of an attachable model do not “find” it, and are bound to the origin of the attachable instead (around the player neck level).
There are no attachables spawning emitters bound to them in Vanilla, so I made a small addon to demonstrate the issue with a custom item:
The flames are spawned from an Animation Controller, and the smoke from an Animation, both implemented on the big torch attachable.
