-Make a portal to the Nether and travel through it.
It may require the player to try this a few times, as it doesn't always happen every single time.

Could this be specific to the location of the nether portal? Does it still happen if you use a different world or move the portals? Could you include a copy of the world save so we can try and reproduce the bug?

Are you able to provide a test world for the issue please? It could be a new world with the same seed and the portal built in the same location, if that would help.
