Loading Game Causes Death

When this issue started happening, I assumed that the game had began simulation before I could react/see/input/etc. Maybe it was a creeper? But every other time?

At this point, each and every time I save & quit I empty my inventory and hotbar to their own specific chests.

$75 on this game (Not including gift copies)... ffs
