you can die loading up the world sometimes you spawn next your bed sometimes you spawn at spawn but your item alway be at spawn.

I too have now experienced this twice. Both times, with 30+xp I have saved and closed a game having returned to a completely 'safe' loacation. Each time when I have returned to the game and loaded my world, I have found myself dying. No obvious cause, nothing found to recover.
