when i start the game up again and play, i cant use item/place block. when i look at the controls, it has been changed to a broken texture/unknown control
i can fix it but its just going to break again next time i load the game

as you can see in the screenshots, it's worse then that. after reloading the game again, 3 controls are now broken.
