its happened to me three times just trying to kill the ender dragon and tons whilst just playing the game somewhere in the dozens
This particular bug keeps hitting me also in just single player while connected to Microsoft and Nintendo. Very annoying especially on top of disappearing villagers and other mobs...

I get this A LOT. and I have never seen it while playing single player, it ALWAYS happens when connected to someone through xbox live. I have counted a total of 10 crashes. it's the same thing, what it looks like lag, and then the entire software crashes. and it's not in the intense moments either. one time I was sitting afk (away from keyboard. or just me putting the controller down and doing something else) and it crashed. I am not happy about this bug. Whatever i am doing at that moment is halted and I have to restart my game in order to keep playing. this bug started with the recent 1.12.0 update that was suppose to be a bug fix update.
