When you save and quit the game on top of a chest. When you rejoin you will take damage losing 4 heart in half’s meaning losing 2 full hearts if around the chest is blocked.

I am not able to reproduce taking damage in Windows 10 or iOS. I did notice that when you relog while standing on a chest, it puts you inside the chest. This seems to be another case of MCPE-59428, which is fixed in the current beta version.
Are you sure that the damage you took was not due to being hit by a monster before the game loaded? A longer video showing the full sequence of save & quit, then reloading the world, would be helpful.
