When villagers summon am iron golem the golems are not checking that there is enough space for them and they are spawning inside the floor. I have seen them spawn in an empty air block with a bottom slab on top, inside bottom slabs with their head inside a top slab, and recently with his bottom 2 blocks both inside dirt (see screenshots).
