When i was in the nether, i threw an ender pearl and then i died (because i was at low health), but not only did i die, but all my items just simply disappeared (yeah! Just like that, POOF!) even though i wasn't near any lava nor fire. Please fix this.

Oh yeah, it was version 1.16.40 but apparently that doesn't exist. On my ps4, it clearly says "v1.16.40"
Were you quick enough to pick up your items? If you don't pick them up in 5 minutes after your death, they will disappear.

Do you mean that you died from the random damage cause by the pearl? Did you check area where you threw from and the area where you tp'd to for your items?
