I play on Bedrock Realms and I tried changing my Respawn Radius but every time it goes back to 10

The setting is controlled by a gamerule. Game rules reset after 20 minutes of there being no one on the Realm. If you have cheats enabled then you can set this with command blocks. I have a series of command blocks (in a ticking area) setting this and more every 20 minutes (using a tick delay). Good luck.

This only happens if you play realms. When I play regular multiplayer worlds without cheats it changes but not realms

We are working on a fix for this, but in the meantime you can download the world, set the game rule, then re-upload the world to your Realm

The setting is now being saved properly and is behaving properly in-game, but the UI in the menu is not updating properly, so it appears as if the value was not saved. We are working on a fix for this, but in the meantime it is now working properly 🙂 Note that if you change any other setting in the menu without typing the correct spawn radius in, it will reset back to 10, so be careful or reset it as needed. We'll have a fix for the ui soon!
