Uploaded existing world to Realms. Hostile mobs spawning is vastly reduced, for example, pigman farm high above the nether is not spawning pigmen. F3 shows very low entity count.
When running the same world locally (form the copy I uploaded to Realms), hostile mobs are spawning.
Render distance is set to 12.
Difficulty: Hard.
Spawn Monsters: On
This occurs with only 1 player in the realm.

I have this issue too. I have a big platform above the nether, right against max height, so no close spawnable areas. When I arrive, pigmen spawn, but they will not continue spawning once those initial pigmen are dead (despite the platform being 32x32). Entity count is near 0. I'm the only person on the Realm. I downloaded a copy of the world, and pigmen spawn fine in singleplayer. I believe it's a realms render distance thing? I have no solutions, and am wondering if I should file my own issue about it, since this one is closed (unresolved)?
I believe this is a render distance issue. Realms is functionally crippled. My understanding is that the server side render distance is set to 10. This cannot be changed by the user. Setting your own render distance higher than 10 has no effect. Presumably done to reduce the processing power used by a realm. I now use Hamachi by LogMeIn to host my own server. Realms, as currently parameterised, does allow full gameplay.
Mick, I believe you're right and it's a server render distance problem and is persistent in 1.12 and 1.13. It's a shame, because I hoped Minecraft Realms' extra cost would allow it to be more functional and easy to use and have more support than a standard server-hosting site. Hopefully they fix this, or I too will be switching away from Minecraft Realms.
Tested a pigman farm on render distance 10 on single player (an exact copy of a world on realms, created in 1.13) and the pigman seem to spawn as they normally should. It isn't any noticeable difference from a render distance of 12. Render distance of 8 causes a noticeable change in spawns. If the render distance is 10 on realms, it has to be something else... ❓ - but it is definitely something about realms. Realms preset render distance is not 8... is it?!
Update: Spawning was working perfectly for the past 48 hours. Now this bug is back.
Pigmen and hostile overworld mobs were constantly spawning when the player was immobile and the mob cap was not reached. Today, we are back to mobs no longer spawning if a player is not moving around.
Whatever you did, please do it again.
I started to design a Guardian farm which meant the player's AFK spot was at 166 to unload all the mobs under the oceans and far away.
Worked really well in single player, went ahead built it on the Realm in survival and spawns are shocking, doing /say @e showed I have a lot of entities not despawning. I killed all hostile mobs using commands expecting the spawns to start, only to find most hostile mobs spawned in the world.
@e[distance=..160] was low but even after a kill command, I had skeletons spawning over 200 blocks away taking up the mobcap
