Light additional info from my own experience: This seems to only happen if player death is within honeycomb fields during a soul storm.

Update: Found a temporary fix to the potion issue by starting a side quest.

I can also add on a far more major issue to do with the Skulker Claws Unique: It can even cause other artifacts to not really work, luckily there's a temporary fix for this by just un-equiping and re-equiping the artifact. more importantly, however, is the fact that the claws can also cause HEALTH POTIONS to stop working. Permanently. Across all sessions. Restarting doesn’t fix the issue. Neither does un-equiping and re-equiping your entire inventory. Even with a piece of armor that has bottomless brew on it, it doesn’t fix the issue.
