you can fix this in 1.16 if nvidia control panel does not work by going to vanilla tweaks and then selecting the item stitching fix and putting it as your resource pack
I if someone has a solution to the entire game that would be helpful because it seems like it's also influenced by the textures scale or something im not a expert

Yeah so I go through any portal in the normal world, and it takes me to this one specific nether portal in the nether. When I leave the nether in any portal, it takes me back to a specific portal that is far away from my base. I tried to fix that by destroying all the portals but I just wasted my time because it didn’t work. I wish I could just reset the nether.
