Please change the platform to “Multiple”. This bug also affects Android and Windows.
It's still an issue in 1.21.101. It seems to happen more frequently when an addon is active.
The report has been edited to fit my findings.
After performing a few more testings, I can now confirm that this bug is very likely only triggered by the Wither drilling a hole in the ground until its boss bar unloads from the screen. At this point, it's highly unlikely it's triggered by something more specific. The first attachment on this comment shows what I initially thought could trigger the bug, while the second and third attachments show what actually does trigger the bug.

So, I think I found something out. This bug seems to happen more frequently when the Wither, upon reaching half-health, drills a hole in the ground until its boss bar disappears. I found out that the player doesn't necessarily have to move away for this bug to be triggered, just like it's shown in the videos attached. Although I'm still not sure if this is the only trigger, or it simply happens more frequently with this.
I'm going to try to look further into this. I’ll let you know what I find out, or at least what I think I find out.

This should certainly NOT be working as intended. More than one wandering trader spawning at once can get annoying pretty quickly.
This is totally not fixed. It's still an issue in 1.21.92. My sister just accidentally killed my Happy Ghast with her bow while we were both riding it.
Can confirm this in v1.21.81. Foxes were spawning inside and on the roof of my house so often to the point where I had to place a command block to kill all foxes in the area. 😒
My mistake. It doesn't affect Windows.
