Please readd it to the creative inventory and add petrified birch slabs etc. and stair, fence, door and trapdoor variants and pressure plate and butron.

This is intended. I always save my command block as a hotbar to not have to use a command to get it. 

This is intended. When someone dies and the entity who killed holds a renamed item it shows the item that it is holding in the death message. That way the item cannot be mentioned if you throw it into lava before the entity dies like it should.
