I was just about to make submit a report about this. I can confirm that Polar bears, Strays, Husks, and Wither Skeletons are missing their subtitles.

This started happening to me in my world when I upgraded from 1.8.8 to 1.9. I'm running 1.9.2 and it's still happening.

I've had the same problem in 1.8.8. It is undescribably deep-cuttingly infuriating for me. I've had countless villagers and important name-tagged passive mobs randomly disappear on me. This has been happening since I can remember in Minecraft 1.8+

I've had this same problem just last night. My friends and I are building a mini-game server out of redstone. Anyways, we got some pretty good work done. Next morning my friend calls me up and tells me none of it saved! I was able to revert to a backup to save it, but then it happened twice more...and it's not corrupted chunks. I had corrupted chunks in a singleplayer survival world and fixed them with MCEdit.
