It appears to have been fixed with 14w08a from what I can tell.

It's probably related to MC-119, but this is triggered by a world reload and happens every time.

I thought the end portal issue was a teleport offset error.

What? It has nothing to do with end portals.

This is just a fluke, not a bug in the game.
