I had this issue too for a couple of days. Luckily the server owner was a close friend so we could try many things.
Resetting the server to an older backup (before the bug occured) helped fix the problem. Without resetting the backup it seemed impossible (tried all the other things people suggested) but as soon as the server owner tried to switch on cheating (to get the other players their progress back from the last couple of days) I could log in again, even on the most recent version of the server.
It seemed the game thought I was logged in as there where a lot of arrows in the ground next to me when I spawned. So skeletons must have tried to shoot my player entity.
