This issue could happen on Realms. And it can be reproduced. Just pull the lever and the zombie should attack. If the Zombie doesn't attack, leave the world and join back and pull the lever again. Then the Zombie will attack.
This could happen in Realms as well. And this issue can be reproduced. Just feed wheat to vows and you should hear no sound. I found this only on MC Bedrock Edition.
