Just happened to me on 1.17.1 vanilla died in nether and 2 min later I arrive at corpse and no items.

Nobody else is in the nether and I was only person online

looks normal to me, your hitbox is inside the other so it fails to place

Any idea how we can prevent these clients from connecting?

Would setting the world to require the exact client version work?

I'll mention this.... you can't import a java world by packing it into a mcworld format.

Also if you edited the world, added or removed experimental gameplay, then packaged it, that too can corrupt it.

I see similar thing on 1.14.20 & 1.14.30 where sometimes the initial spawn for the obsidian pad will generate below Y=50 while the bedrock for End Dragon is around y=60.

And sometimes after killing the End Dragon if you return back to the End from Overworld it will replace the obsidian with Endstone and spawn it below y=50.

Doesn't always happen, and if we delete the chunk data that seems to fix it.

But have never seen it spawn in without an initial pad though

This is normal behavior since there is no stronghold to point to.

Same thing happens in previous versions of flat worlds

Note: If you have a normally generated world and destroy the end gateway blocks in creative, the Eyes of End will still point to that area, even though the portal was removed.

I have seen this issue if the Steam Client is running while playing a game that has native XINPUT.

Close your steam client before playing games that have built in support for XBOX controllers, unless ofcourse you are playing a Steam only game 😉

Tested with different configurations allowing more time to rise and it still drops of falls short.

As if the timings of redstone signals are way off compared to Java
