I've been able to recreate the bug very reliably by entering the eyes menu and selecting any option from the lower half of the available options fast enough. It occurs with most menus I believe, I've recreated it on the torso, overwear, back items and hair aswell as eyes in the past but it's most prominent and easiest on the eyes tab. I don't believe there's a consistent fix for this yet but in the past, deleting and reinstalling the game has worked for me (which is how I've recreated the bug over and over).
For nintendo switch and android beta: The bug also cannot be solved by deleting and reinstalling the game however I have in the past been able to fix this issue myself by creating a new character in the corrupt slot before you sign into the game or by switching to a classic skin within the same time frame. This is very unreliable however and most of the times will not work and will instead overwrite your next available, working character. I haven't tested this method when all character slots are corrupt.

I signed into my account normally expecting the bug to just cause my character to not load however instead it loaded a new default skin in the slot and it became fully functional again. This did fully fix the issue but there's no way to replicate it as far as I know since I don't remember doing anything special other than signing in.
