While placing blocks, it's possible to swap hands at the perfect time to avoid placing a block, causing the player to sometimes walk off a block when in a crouched position.
Notes:
It seems to be a timing issue, so it doesn't happen with each block
It doesn't appear to be directional, reproducible in each
Swapping with two stacks of blocks doesn't cause the player to fall
Happens if pressing instead of holding buttons down as well
When it occurs, the block place is cancelled? It temporarily appears and disappears
Unconfirmed: Only happens on primary -> offhand swaps?

I highly recommend testing this on glass pane / iron bar parkour. Every time you slip off while crouching, recall if you were backing up when it happened.
It's possible to slide off of the glass pane while crouching if you are slipping forward while attempting to back up while crouching. If you are not backing up, you will not slip off.
We do not have enough information to find the cause of this issue.
