This is related to MC-176919, and caused due to the « fix » of MC-125254.
This is very easy to cause unintentionally in vanilla survival : one only needs to strike a lightning rod next to them and die from the strike.
This could be intended purpose.

MC-206922 is indeed related to this bug report (although it affects the trident item dropped by the trident itself instead of one that could come from the inventory of the player when they die), and the fix proposed @unknown in this comment will probably solve the bug if implemented.
After some testing, I have found that this issue can only be reproduced with a trident enchanted with Loyalty. Since a trident enchanted with Loyalty drops as an item if the player dies before the trident hits a block or mob (see MC-176919), throwing a trident with Loyalty and Channeling at a mob or a lightning rod and dying immediately before the trident hits the mob or the lightning rod causes the trident to drop as an item and then to be destroyed by the lightning that is still present (see MC-206922). Therefore, this issue is a duplicate of both MC-176919 and MC-206922.
