I was digging recently in the sounds.json and found 3 interesting sound groups: "game.player.die", "game.neutral.die" and "game.hostile.die".
I've changed the sound to all of them but they are not present when a player dies. The /playsound command with those 3 works just fine and it is playing my custom sound.

As far as the music.game.end and similar do work, all the death sounds do not. I've updated the affected versions, thanks for reminding.
Like requested, I've posted my sounds.json, it's pretty complex cause I've changed most of the music features, but you can clearly see the 3 death types sounds, none of them is working.
Was poking around in your sounds.json... just out of curiosity, try removing the weight lines from the .die. No reason for them anyways as they are the only sound file, so weights have no effect.
Other than that so far not seeing anything malformed that could be causing you issues. Try to play the sound file then provide your console output.
Please attach the complete output of the "Development console" tab of the launcher.

Is this still a concern in the latest Minecraft version 14w30c? If so, please update the affected versions in order to best aid Mojang ensuring bugs are still valid in the latest releases/pre-releases.
Still an issue in Release 1.8 as ShadedPL last described. This should probably be altered so that death sounds play not only when you kill a player, but when you die by yourself as well. As-is, there's no way for resource pack authors to test these sounds without finding someone to be a meat puppet, which can be a hassle.
Is this still an issue in the most recent versions (currently that is 1.10.2, or 16w42a) of Minecraft? If so, please update the affected versions and help us keeping this ticket updated from time to time.
