Confirmed. Multiple entities or players rapidly enter into the nether, and players will be redirected to another portal which is not intended to be linked. Private server available if demonstration is needed.

Second, but you added some valuable information to the existing issue.
Cannot reproduce, just grew a small jungle on superflat, all had leaves.
