The Bug
Putting two undamaged items of the same kind into an anvil and changing the name, will allow taking out the combined item however, the second item is consumed without any benefit. This is not possible without renaming.
How to reproduce
Place an anvil
Get two new helmets of the same type (or any other two items that can combine this way)
Open the anvil and put both helmets in
Type something else in the rename field
Take out the renamed item
→❌ Notice the second helmet is now gone

It has been confirmed. Another problem is combinding enchants together in an anvil to make a master book. It has been messed up in the 1.5 update, but will probably soon be fixed.
