Could this maybe be this MC-62958 ?
With this bug, very strange things can happen, also mob and maybe player models can get glitchy
Also please don't report 3 times a similar bug, instead report it once and add there all the screenshots 🙂
Please force a crash by pressing F3 + C for 10 seconds while in-game and attach the crash report ([minecraft|http://hopper.minecraft.net/help/finding-minecraft-data-folder]/crash-reports/crash-<DATE>-client.txt) here.
Ticket resolved as incomplete, because no answer in a reasonable amount of time (1+ year), if it still happens, please update the ticket.
