[MC-88779] Server kicks players when activating command blocks

When I activate a command block chain in my multiplayer server (when more than one person is online), one of the players is kicked from the server when the command blocks are activated and they get the following message:

I saw a similar issue here: MC-86933
but I do not believe that the issue is the amount of ram used. I allocated 4 GB for the server, and 2 GB for each player. (I have 8GB on my computer.) I would appreciate any advice or updates on this issue.

Here's the server log: note that I cut some of the "[Server thread/INFO]: [@: Score 0 is in range 0 to 0]" since it was redundant and unnecessary.

A similar issue was fixed recently. Is this still an issue in 15w44b? If so, please explain the issue in detail and provide steps to reproduce it. Please also update the list of affected versions.
