Putting an item into an enchantment table without enchanting it will some times duplicate the item. Also if the player has an enchanted item in his inventory while trying to enchant another item it will get rid of the enchantments on the item that was already enchanted.

Just because you haven't had it happen in beta doesn't mean it doesn't happen. You probably just want it to stay in the game so you can duplicate your items. It happens for me all the time.
