This has been a bug for as long as I can remember, so basically what happens here is that items that are in your hand are slightly rotated to the left (or right, depends on if it’s on the offhand). Blocks are not affected by this.
I left some attachments with text showing which version is which for you to easily notice the difference.
Steps to reproduce:
Hold any items in either hands (cant be a block)
Go in 3rd person front
Look closely at the item
Expected Results:
The item should be facing straight forward.
Observed Results:
The item is slightly rotated.

I think this issue is exactly the reason of why MCPE-17366 happens.
This issue may be also the cause of MCPE-148177.
Please go vote for bug report MCPE-95929!
It only takes a second.
We need more votes to get Mojang to fix it.Thank you!
The fact that items aren’t properly positioned in the player’s hand really frustrates me. Addons that use the right item bone to display items (for example a pedastal) have to correct this issue manually, which is very frustrating.
This bug was mentioned in the 26.70.21 changelog, but the listed change actually does not fix anything in-game.
Here are all the item poses that are still not aligned with Java Edition that I could find. Most of these items are also incorrect in first person!

Items marked with * are incorrect due to attachable animations, which require a whole different approach than items without them!
