Part I, with horse
Load https://bugs.mojang.com/secure/attachment/550787/MCPE-170425.mcworld
Ride the gold horse into the nether portal, dismount, and wait to be transported to the nether.
Once in the nether, place a boat on the longer ice path and ride it to the end.
Part II, with any mob
Load https://bugs.mojang.com/secure/attachment/550787/MCPE-170425.mcworld
Go to overworld 0 58 -14.
Spawn a passive mob and push it into the portal
Go into the portal.
In the nether, place a minecart on the minecart track and push the mob you spawned earlier into the minecart.
Push the minecart all the way down the track to the other end. (Takes a few minutes.)
Break the minecart to release the mob.
Push the mob into the nether portal at -416 18 -352.
Go into the nether portal.
Wait for the mob's portal cooldown to expire, then push the mob into the portal on the overworld side.
Once in the nether, place a boat on the longer ice path and ride it to the end.
Expected result
When you load into the nether at -419 18 -349 the gold horse from Part I or mob from Part II is there with you.
Observed result
You find the gold horse from Part I or mob from Part II near the portal at -6 18 -16.

This is likely the original issue behind MCPE-22340, and one of the issues commented/duplicate-linked on MCPE-66818. I am not sure if the repro with horses requires that the horse had ridden a minecart through the nether at some point as in the repro with any mob. Once a horse is affected by the bug, it continues to exit from the wrong portals even after relogging. I believe the same issue affects donkeys and mules, for example see REALMS-5403.
I was able to reproduce this in a brand new world with the horse coming out of a different portal within sight of the players':

This also happens if you push the horse into the portal or pull it in with a lead instead of riding it in and dismounting. Unfortunately, I do not recall if I ever put this horse in a minecart.
If you continue with the mob from the part II repro and repeat steps (5) - (7), then the mob will disappear. It is actually teleporting to the portal nearby which it should have come out of in step (10) the first time through. If you then go through that portal you will find the mob in the overworld. The issue is directly related to processing capacity, if your device is not very fast you will be able to reproduce correctly. The original author used a Nintendo switch and Nintendo switches are not very good with Minecraft due to performance problems.

I still cannot reproduce on my older PC (i5-6500 processor and GTX 750ti graphics) after setting simulation distance to 12 and render distance to 32. However, I reproduced MCPE-170425 with those settings when trying to reproduce this bug.
