The Bug:
The new play screen did not appear for me in the 1.21.51 Hotfix. This addition was specifically mentioned in 1.21.51 Hotfix changelog.
Steps to Reproduce:
Update to 1.21.51 Hotfix
Click the "Play" Button.
Notice you may have got the old play screen UI.
Expected Results:
The new play screen should have been added in 1.21.51 Hotfix, as mentioned in the changelog.
Observed Results:
The new play screen has not been added in 1.21.51 Hotfix even though it was mentioned in the changelog.

According to the changelog:
These updates are being rolled out incrementally, so they might not be available right away.
Which means that these changes have not been fully implemented yet, and it'll take a while for everyone to receive the update.
This issue is invalid. The 1.21.51 changelog specifically mentions that the updates to the play screen are still rolling out and will not come to everyone at once.
