While flying around in the overworld I noticed it's possible to fly through blocks from underneath and take suffocation damage.
It's very apparent in the nether. Try flying into the ceiling, maybe an angle where you fly upwards into a corner. I pass through the block and take a couple of hits of suffocation damage. I've always immediately directed myself downwards to fly out. I've not tried to stay in the block to see what happens.

This still affects 1.9, and is very easy to see in nether tunnels with flat, solid block roofs. Fly along, and your head will clip through the ceiling dealing damage to the player.
