Summary:
In Survival mode, bone meal is not consumed correctly when used on grass or ferns. A single bone meal can be used several times before it is actually consumed.
Steps to Reproduce:
Place down grass or ferns.
Give yourself bone meal.
Set the gamemode to Survival.
Use the bone meal on the grass or ferns.
Observed Results:
When the bone meal is used, it is almost never consumed and must be used several times to consume it.
Expected Results:
The bone meal is always consumed when using it.

I'd also like to add that rarely, in certain areas, bonemealing fern does not result in large fern, even if there's open space above.
@unknown: I was not able to reproduce that in a flat world under 1.16.210, but that may just be because it's rare and only occurs in certain areas. Can you be more specific? Do you mean specific biomes, for example, and if so which ones? Does it miss once and then work the next time you try, and if so how sure are you that you didn't simply aim wrong? Or does it simply not work for some ferns no matter how many times you try? Most importantly, what version and platform are you playing on?
