10-player realms are experiencing a lag when 3+ are online, if a player enters a Nether Portal, it will 9/10 times cause the realm to disconnect every player online. If it doesn't, the realm will experience a 5 second freeze where the player can still more, but nothing in the game can be interacted with nor chat message sent or received. This occurs around every 30-40 seconds.

Also, are you able to provide a copy of the Realm world save for us to try and reproduce the issue in a local game?
You will need to download a copy of the Realm world, then:
On Windows 10 you can use the Export world feature.
On Android or iOS you need to use a file explorer to navigate to Device storage > games > com.mojang > minecraftWorlds
On Xbox or Switch the only way is to upload it to a Realm, and then download the world on Windows 10 or Android/iOS.
If the world is larger than 10MB you can upload the world to OneDrive or a similar file sharing site, and then share the link.

Additional troubleshooting, I have disabled every command block within the world (enabled in the download) and the problems still persist on the realm, location is 10000 251 10000 if you want to check those out.
As for local host replication, I have been unable to reproduce the disconnects or the lag experienced on my XBox. We were going to attempt to upload onto another realm to see if it maybe the hardware within the virtual server host our realm sits in not scaling as it should, all in hopes to see it if is a hardware issue or world issue. Connection wise, all our members are within the US or Canada, one South America.
Again, thank you for your time and efforts. I will do my best to provide you with any information needed to help resolve the issue and help determine the problem.

What we can determine so far:
With 2 online, regardless of location, we have zero lag. If you take a portal, players will experience the freeze lag, but no disconnects.
When 4+ are online, we experience a jittering lag.
When a player, or you, go through a portal with 3+ online the entities will freeze in place, can't chat or open opens, etc. However you can move around. THIS HAS CAUSED A REALM WIDE DISCONNECT MOST OF THE TIME.
Is noted that if the realm does not disconnect after a Portal trip, the game begins to freeze lag.
We do experience a lag upon members joining, however, that is normal.
Loading myself millions of blocks from spawn did not yield any noticeable lag for the other online.
I have had had reports of only one online, after 12am EDT, and they have randomly disconnected and well as disconnects when just two mining and no portals were being traveled.
Realm Back-up on Single Player
No lag or portal caused disconnects.

I want to update.
I disabled the system at 10000 251 10000 marked Detect by Spawn and it seems to have resolved the Portal Crashes. Wouldn't call it fully resolved, I had 3 Admins go through the Portal at the same time, with 2 others online, we still had a Realm-wide Disconnect, had to Close Realm and Open Realm to get people back on again.
[Edit] - Had a 4-5 second freeze when someone went through a portal into an unloaded area. Had 4 people on at that time, one at spawn, two in the End and one coming out of the Nether into the Overworld. Logged a 4-5 second lag on mobs/chat/chests/etc.

Any chance we looked at the world save?
We are still having the issue, but it isn't as bad.
Does it have to do with world size?

Hi Dave. Sorry to hear about your issues. The easiest way to proceed is it you contact support at https://help.mojang.com/customer/portal/emails/new . This way we can verify your ownership and proceed to check logs and interact with your world to figure out the problem.

I have submitted a ticket for this issue via this process and I just got a blanket response that made no attempt to resolution beyond just going to the next e-mail.

I am sorry to hear about your problem.
There are a few things you can try to fix this issue:
Make sure that you are connecting through the default app.
Try connecting to your Realm from a different network.
Try accessing your realm from a different device.
Restart your device.
Make sure that you're logged in with the same account (Apple ID, Google Play etc.) as you made your purchase with. You might want to log out and log back in.
If you are the owner of the realm, please try the below suggestions. If you are not the owner, please contact the owner and ask the to try the following:
Try downloading a local copy of your world and then uploading it again with the replace option
Try reverting your realm to a previous backup from before the issue occurred (Make sure to save your current world first)
Remove any texture packs from the worlds, and try again
