Okay - it seems to happen when certain color codes are sent to the client - still random
Also interesting is it is not there on 1.5 but is on 1.5.1
I am not sure exactly what is happening - but i managed to achieve this bug on a vanilla server (not sure how i did it)
Surly this is a client issue though
Was this a public server (do you ever have any players other than you and your friends) and was the <Username> someone you know?
Do you host the server or do you use a hosting company? (i include through a VPS as you - i mean a hosting company like minecraftserverhost.net)
EDIT: so the player did not actually join - it just said that they left
same here F3+C does NOTHING AT ALL! annoying because i have this bug LOTS and would like to help - STUPID F3+C!
i am not very good at JAVA but i think this was because it tried to load item with ID #142 (potatoes)
were there any MODS on the server (was it un-modded)
EDIT: also tried to load ID #140 (flowerPot)

Why dont they make them "Blocks" (also - what is an entity - is it like the dropped items and players and mobs?)
UI bug? - do you mean GUI?
Vanilla server spawn is always close(ish) to a centre point - i think spawn protection protects around the large spawn circle (never spawns in 1 place) to fix spawn - use command block with location permitters setting anyone in spawn points spawn to 1 location with /spawnpoint

@Kumasasa - why does it let you select future versions?
tips to remove lag:
1. No texture pack
2. View bobbing - off
3. Particles - minimal
4. Graphics - fast (i think)
What are your computer specs (RAM / free memory (storage) / RAM you let minecraft use (other details))
DELETE ALL MINECRAFT FILES AND RE-DOWNLOAD LAUNCHER - THIS SEEMS LIKE A PROBLEM WITH YOUR COMPUTER (sorry about caps)
Have you deleted your .minecraft folder if you have deleted it - delete the folder again and re-install the launcher(your launcher may be broken) - are you using the launcher provided bu Mojang?
can you update to 1.4.4?
I had this issue on my own offline server - i had NO torches ANYWHERE! - i had a HUGE and annoying issue - i tested this with sphax+optifine default+optifine sphax+no mods and default+vanilla
Why does this reporting system let you mark it as a future version?
I get this on my localhost server but same error.
ALSO: None of the numbers match the email AND i logged on properly.
