@"Andrew Ng" have you verified they are not at 0 durability? are you able to fly with them in the overworld? please provide more detail as to exactly what you've tried, what you're doing in case there's something about that that's causing the issue.

I found this (w/ end portals) in 3 successive worlds... (and naturally generated end gateway detail below as well)
does not happen in creative, only in survival
in each, i created an end portal within ~ 10 blocks of spawn, went into survival and hopped in...
the first consistantly sent me to -457 70 522 (out in the middle of the void... )
the second consistantly sent me to --2 75 -18 (in the air around the starting end fountain/exit portal)
the third consistantly sent me to -100 49 0 (in the air above the obsidian platform... enough to take about half my hearts in fall damage)
I found if i went in creative first I'd start on the platform and be teleported to the above coords
I also found end gateways (the single block ones embedded in bedrock) are similarly bugged, upon retrun to the main island, the one i tested placed you inside the bedrock gateway, meaning you could only escape in creative... otherwise you'd take a tick of damage and be returned to the other end
confirmed in 15w39b survival single player, new world, Windows 7 64 bit w/ 64 bit java, intel 2600k cpu, nvidia gtx 970 gpu.
the error from MC-65040 "Tried to add entity X with pending removal and duplicate UUID xxxxxxxx-xxxx-xxxx-xxxx-xxxxxxxxxxxx" DID occur in the log, but not with regard to said invisible skeletons (it was about some villagers that were likely at the edge of loaded chunks I'd flown in render-range of)... and the above error was 6 minutes before the "was shot by a skeleton" death message that preceded my respawn and subsequent invisible skellies
I also noticed and tested, if you lure the invisible mob out of the chunk they went invisible in, they become visible again. I hope this is relevant and helps solve the issue.
in this snapshot can confirm not only invisible zombies but other mobs and invisible dropped items... you actually get shot and/or hit by un-rendered mobs and items dropped on death are also invisible but collectible (you can walk over the area where you'd died, see nothing, but your inventory will fill up with your old loot). I suspect some of the eliminated chunk render cycles were for rendering entities.
I appear to be seeing this as well... was looking for a "disappearing anvil" bug, but this would be as good an explanation (my anvil was embedded in the floor so I didn't notice degradation before it disappearing on the 5th or 6th use) I shift clicked a chest plate out of it (combining a diamond protection 3 chest w/ an unbreaking 3 book) and the anvil was gone when the ui exited (due to the anvil being gonel)
I'm seeing the same issue it seems, crash to launcher, no consistent asction in game seems to cause it.

additional detail, this is a world I'd played for 4-6 hrs already and this only happened after a save/exit/reload cycle. No complex redstone or command blocks... just 5 pistons, 2 repeaters, some dust and one redstone torch keeping the pistons extended.
I was near a village when it happened, also near an enchanting table, some growing crops, during day.. had recently used the bed.

this is not the block duplication issue either. Each placed rail is removed from inventory, and can be collected later. They're just a.) allowed to be pushed out over air blocks, and b.)not functional* while over air. (and under some circumstances I haven't pinned down, the pushed rails are not functional on land either)
*by functional I mean they'll neither link to existing rails or carry a minecart.
MC-661 is a version of the floating torch bug marked unconfirmed, as well as a duplicate of a bug https://mojang.atlassian.net/browse/MC-2512 that was resolved in Snapshot 13w06a. This has been confirmed in 1.5 pre-release. (while the torches on the one lamp post I've seen in that version were not floating.)
Confirmed in 1.5 pre-release can only combine enchanted books of the same type and level in creative, but not survival.
it appears so. block disappears on reload and update as well. (also caught the piston "deleting" blocks,like the video in that ticket) so please feel free to close and link. Sorry for the dupe.
On further inspection, the created block (farthest from piston) Is solid, and can be stood upon and cannot be walked through from the side, but does not drop a block when mined. I suspected it could be a block id conflict with 2 instances of the duped block (blackbox test assumption) so i duped the block in survival, switched to creative, removed the block that would have dropped a birch log item wen pined, switched back to survival, and tried to mine the dupe block, but it still did not drop anything.
to duplicate in survival mode (and be able to mine the blocks "in flight") I used Birch logs and a diamond ax with "efficiency V".
started with a stack of 64 Logs, screencap shows 63 on hotbar + 2 on piston after placing 1 that got duped to 2.
can confirm in 15w45a. dropped boat on to crystal, entered the boat. the first time I got a crash upon exiting the boat, the second crash was while paddling. (I'm using win 7, java x64, gtx 970, 2600k cpu, 8Gb of ram allocated to minecraft)
