I've checked 1.18.2 and it looks like the issue have been fixed. Writing to disk is back to normalish levels, also less stutter, less HDD noise.

I've attached minecraft performance report in starter post, as well as added screenshot of System Explorer monitor that shows basic stuff like total CPU usage, minecraft writes, etc

Generated on fresh clean 1.18.1 game and world while walking in direction away from spawn, default settings.

Excessive writes causing lots of stuttering, if it does not write to disk - game is smooth.

FPS in 1.16.5 - 65 fps

FPS in 21w15a - 61 fps

Windows 7 SP1 with latest updates

I'm thinking about opening bug report of similarish fashion, on 1.18 singleplayer I got insane disk writes from minecraft, by just walking I can top it to 2mb/sec of constant writes into region files and HDD is just crying. This is accompanied by lots of stuttering. Versions prior had very low level of writes and much smoother (1.17 for example, you notice it right away)

Affects 1.15-pre 2 IMHO. Played on multiplayer server, traversed about 200 blocks to spawn camp, it had pretty much clean terrain with a micro farm, mine with stairs and portal and minor grief.

This bug was noticed during the night while removing grief, standing on flat surface with few torches, placing random blocks at ground level created almost 50 ms draw spikes (very noticable when moving), blocks were placed away from chunk borders. Was unable to reproduce after restart, but torch spikes varied from okayish to ~40ms in extreme cases even after restart.
